Initial Visit

When you arrive for your first visit, you will complete a medical history form as it relates to dentistry & a HIPAA form to protect your patient information. Next, we will take any necessary x-rays, conduct a thorough examination, including a clinical exam of your teeth, gums, & other soft tissues of your mouth.

Your treatment agreement will include applicable fees and you will be informed of all costs before treatment is administered.
