Initial Visit
When you arrive, you will complete a medical history form as it relates to dentistry and a HIPAA form to protect your patient information. Next, we will take any necessary x-rays, conduct a thorough examination, including a clinical exam of your teeth, gums, and other soft tissues of your mouth.
